Levels of Care
Complete Care at Home employs certified and trained caregivers to ensure the right match for each client. We assess care needs in person whenever possible to align the caregiver's abilities with the client's requirements.
Our goal is to reduce hospital readmissions by collaborating with all care providers during hospital discharges. We personalize care by considering the client's interests and tailoring services accordingly.
Our certified caregivers are matched to each client's unique needs, ensuring personalized and compassionate care.
Care Level 1
Companion Care (Standard Level of Care):
This level is for those who need a companion to ensure safety and provide company while maintaining independence in daily activities.
Care Provided:
- Assistance with meal preparation and cleanup
- Light housekeeping and laundry services
- Help with shopping
- Pet care
- Participation in outings
- Transportation to appointments and outings
- Medication reminders
Care Level 2
Minimal to Moderate Assistance:
This level is for those needing more support than Care Level 1 due to challenges like weight-bearing issues, falls, weight loss, or forgetfulness. Care focuses on providing minimal to moderate assistance with daily activities while ensuring the individual remains actively involved.
Care Provided:
- Assistance with ADLs (Activities of Daily Living)
- Meal planning, preparation, and cleanup
- Light housekeeping and laundry
- Dressing and grooming
- Shopping assistance
- Incontinence and toileting care
- Pet care
- Transportation to appointments and outings
- Medication reminders
- Ambulation and transferring support (stabilization/assisting devices if needed)
Care Level 3
Moderate to Full Assistance:
This level is for those needing more support than Care Level 2, providing moderate to full assistance for individuals who can bear some to little weight and require stabilization or assisting devices due to weakness.
Care Provided:
- Specialized diet and meal planning, preparation, and cleanup
- Light housekeeping and laundry services
- Moderate to full dressing and grooming assistance
- Moderate to full hygiene assistance (bathing, oral care, hair care)
- Moderate to full incontinence and toileting care
- Pet care
- Transportation to appointments and outings
- Medication reminders
- Moderate to full assistance with ambulation and transfers (stabilization/assisting devices to full non-ambulatory transfers)
- Customizable required services
Care Level 4
Disease Specific or Bed Bound Care:
This level is for those needing more support than Care Level 3, including disease-specific or bed-bound care, with full assistance for non-weight-bearing individuals.
Care Provided:
- Specialized care for chronic conditions such as Parkinson’s, Alzheimer’s, dementia, and confusion care
- Catheter care
- Colostomy care
- Making telephone calls for the client
- Oxygen concentrator and tank management
- Urine specimen collection
- Nebulizer assistance
- Specialized diet and meal planning, preparation, and cleanup
- Light housekeeping and laundry services
- Full hygiene (bathing, oral care, hair care)
- Moderate to full dressing and grooming assistance
- Moderate to full incontinence and toileting care (if applicable)
- Pet care
- Medication reminders
- Transportation to appointments and outings
- Customizable required services
